The two-question review

A one-page after-action review for small misses: the late report, the contact that went sideways, the snap at a partner. Use it with a deputy, a trainee, or yourself. It's not a disciplinary form, and it's designed so people stop hiding their mistakes.

After the miss

Ground rules: Ask it the way you'd ask someone you respect and want to see get better. No verdict on the person. Hold the bar high, and don't beat the person reaching for it.

1. What happened?

Facts first, in order. Then: what moved first, before the decision? Where was the gap?

2. What do I want to do differently?

One specific change. Then: where can I practice it on a free rep before it matters?

The repair

Does anyone need a repair? Write it with no "but" in it. Do your part, then let them have theirs.

Come-back time: from the miss to the honest repair was ______ (minutes / hours / days). The goal isn't never falling. It's coming back faster each time.

For supervisors: run it on your own miss first, out loud, in front of your shift. Nothing makes it safe for a deputy to be honest like watching the sergeant go first.
